Our New Name

After 30 years, we changed our name!

 

On May 1, Hospice and Palliative Care of Contra Costa became...

 

Hospice of the East Bay

 

Our hospice has grown rapidly as more people have become aware of the value of hospice services in general and of the expertise of our hospice in particular.  In 2006, almost 1,500 patients and their families benefited from our care.

 

For many years, our service area was limited to Central Contra Costa County.  Now, our pediatric program, Comfort for Kids, draws patients from Contra Costa, Alameda, Southern Solano, and San Francisco Counties.  In addition, we are caring for more adult patients in Alameda County, and we are witnessing a significant increase in requests for our services in East County.  The name Hospice of the East Bay reflects the expanded area that we now serve.

 

The Hospice of Contra Costa Foundation is now Hospice Foundation of the East Bay.  Information on either agency will be found at our new web address:

Hospice is a specialized, personalized approach to medical care and support at the end of life.  Care is focused on honoring patients' wishes, ensuring their personal dignity, and providing physical, emotional, and spiritual comfort.

 

Since 1977, Hospice of the East Bay has been effective in improving the quality of life for adults and children with life-limiting illnesses by providing expert, compassionate care in the comfort and privacy of home.  "Home" may be a patient's residence, that of a friend or relative, or an assisted living, residential care, or skilled nursing facility--or our Bruns House inpatient hospice facility.  The goal of care is to make our patients as comfortable and as free as possible from pain, loneliness and fear.

 

At our hospice, ALL medically qualified patients receive services, regardless of their insurance status or ability to pay.

 

Family members of our patients are trained to confidently care for their loved one, with the constant support of our hospice team.  They also are assisted as they cope with their own emotional, spiritual, and practical needs.

Hospice team members are available 24-hours a day, 365 days a year.  It is our mission to be by the sides of our patients and their families, every step of the way, so that they do not feel alone and are ensured the best possible quality of life during the important final phase of life.

 

Hospice Services:

 

Our team of healthcare professionals, led by our Medical Director, provides the following services at no cost to the patient or family:

  • Medications and other methods of pain and symptom control
  • Coordination of care with the patient's personal physician
  • Medical equipment and supplies related to the illness or condition
  • Individualized medical care focused on the comfort and honoring the wishes of our patient
  • Practical assistance with personal care and the activities of daily living
  • Frequent assessment of  the patient's and family members' needs
  • Coordination of community resources and help with non-medical concerns
  • Help for the family with reaching consensus on care options, planning for the future, mending relationships, and easing other emotional and practical difficulties
  • Help coping with spiritual questions and needs at the end of life, either directly or by coordinating services with the patient's and family's spiritual advisors
  • Bereavement services to help patients and families deal with their grief. Bereavement support for family members continues for 13 months after the death of their loved one.

 

Hospice Care

Hospice Care is directed by the patient’s own physician, or a hospice physician, and benefits patients with a prognosis of six months or less who have chosen to forego further curative treatments.
